I hope you had a good summer. After 40 years of private practice, I have decided to wind down my office practice. After much thought, I will retire from my office practice on October 31st, 2026.
If you want any of your Medical Records, any written communications with the office should be mailed to PO Box 40093, Indianapolis, IN 46240 starting immediately. You can get your MRI dictations from whichever facility your MRI was performed at by contacting them directly.
